As the Director of
Health and Wellness, I’d like to welcome you to
Schreiner University. The Health, Wellness and
Counseling Center is located in the Mountaineer
Center. The Center is staffed with an RN 9 a.m.
to 5 p.m. Monday through Friday and provides
basic first aid and assessment of minor
illnesses and injuries with medical referral as
necessary. Educational and wellness programs are
provided throughout the year, and numerous
resources are available to the student in the
Health, Wellness and Counseling Center’s
Conference Room.
The following
forms and records are extremely
important to you and enable the nurse to
help you should you become sick or injured.
They must be on file in the Health Center
prior to the student’s residence hall
check-in.
*Please return these forms and records
in the enclosed envelope or to the above
address or fax to (830)792-7304.
*HEALTH FORM:
All residential and international students
are required to have a Health Form on file
in the Health Center. Complete this form in
its entirety including emergency contact
information, immunization information,
health information and signatures. In your
admission packet is information on Bacterial
Meningitis. It is required you sign the
signature line on the health form indicating
that you received this information. The
Health Form will not be accepted without
signature. Return this form as described
above.
*IMMUNIZATION RECORD: All
residential and international students are
required to have an immunization record on
file in the Health Center. All international
students are required to have a TB test
completed within the last year with
documentation of results. We request your
adherence to our recommendations, supported
by the Texas Department of Health, that
students present immunization proof for MMR
(Measles, Mumps, Rubella), Polio (not
recommended for persons over 18) and T/D
(Tetanus/Diphtheria) or T-dap
(Tetanus/Diphtheria/Pertussis). Other
recommended vaccines include Hepatitis A,
Hepatitis B, Varicella booster, the
Meningococcal vaccine and, for women between
the ages of 11 and 26, the Human
papillomavirus (HPV) vaccine. Complete the
Health Form section on Immunizations or
attach a copy (not original) of your
immunizations and return as described above.
(Exceptions or waivers due to religious or
other circumstances may be made through the
Health and Wellness Center.)
HEALTH
INSURANCE: All
residential, international and
vocational nursing students are
automatically billed for student accident
and sickness insurance on their student
account unless there is proof of
comparable existing insurance coverage.
If you have proof of comparable existing
coverage and wish to waive out of this
insurance plan, you must waive coverage via
the on-line waiver process during the open
enrollment period listed below:
New student
open enrollment: May 1, 2009 through
September 4, 2009
Returning student open enrollment:
May 1, 2009 through September 4, 2009
New student enrollment for Spring Semester
'09-10: December 1, 2009 through January 14,
2010
Health Insurance
is mandatory for residential and international
students as it insures students can access
health care off campus, consult a specialist and
have coverage for emergency and hospitalization.
Costly medical bills can impose tremendous
hardships and even necessitate withdrawal from
school. All students are encouraged to carry a
copy of their insurance card to avoid delay in
healthcare in an emergency and to provide
updated information via the online web waiver at
any time when it changes.
Information regarding this insurance plan will
be provided via direct mailings, during
Mountaineer Days and Monty Days or by visiting
the Schreiner University website at:
